Papers to be required in connection with the various official works related District Inspector of Schools (S.E.)


A)    Prior Permission:- due to Deputation / Retirement / Resignation / Transfer vacancy :-

1.      Forwarding letter mentioning the matter from HM / TIC.

2.      Complete MC Resolution mentioning the case, date of occurrence, if resignation vacancy acceptance of resignation mentioning the complete matter.

3.      Approval of appointment of the incumbent / copy of retention order in additional post with subsequent approval of appointment.

4.      Copy of receipt of B.Ed. college, joining report at B.Ed. College / transfer order      / release order / copy of resignation of the incumbent / copy of death certificate.

5.      Staff pattern.

6.      Copy of vacancy roster (duly verified).

7.      Post sanctioning memo both for normal & HS section.

8.      No litigation certificate / if any court case Cleary to be mentioned the said post.

9.      Validity of MC.

10.  Vacancy statement.



B)    Approval of Panel (Due to deputation vacancy, Ref:- G.O. No. 605 dt. 20.06.06)

1.      Forwarding letter from HM / TIC.

2.      Copy of Prior Permission.

3.      Copy of MC resolution – formation of selection committee, screening committee, Name of proposed expert, date of interview, Name of the daily newspaper for advertisement. Copy of Paper cutting.

4.      Copy of resolution of staff council regarding selection of T.R.

5.      Individual score sheet.

6.      Compilation sheet.

7.      Panel duly signed by the selection committee

8.      Resolution of the selection committee.                  

9.      Non-relation certificate.

10.  MC resolution approving the panel.

11.  No litigation.

12.  Validity of MC.

13.  Supporting document (postal documents) of the call letter.

14.  Copy of attendance sheet of the candidates by duly signed by the selection committee.

C)    Approval of Appointment:-

D)   18 Years Benefit:-

1.      Forwarding letter from HM / TIC.

2.      MC resolution mentioning continuous and satisfactory service with date of joining and date of completion of 18 years.

3.      Prayer cum option of the incumbent.

4.      Copy of approval of appointment.

5.      Copy of option fixation of last ROPA.

6.      Copy of service book.



E)    Countersignature of Service Book / LPC:-

1.      Forwarding letter from HM / TIC.

2.      Approval of appointment.

3.      Transfer order / Release order.

4.      MC resolution.

5.      Service book with Xerox copy – filling all columns.

6.      No litigation.

7.      Validity of MC.

8.      Copy of Acquittance roll – concerned month, previous month.

9.      SSC recommendation, Certificate of academic qualification.

10.  Option and fixation.

11.  Copy of LPC.



F)    Arrear Salary:-

1.      Forwarding letter from HM / TIC.

2.      Bill format as per G.O. No. 3049 of 04/12/2006, Annexure – I is to be signed by the HM / TIC.

3.      MC resolution (Cause period, amount to be mentioned and duly accepted by the MC).

4.      Approval of appointment.

5.      Court order if any.

6.      Copy of service book.

7.      Copy of Acquittance roll for the concerned period.

8.      Copy of prior permission if necessary.

9.      Certificate of qualification / mark sheet if applicable.

10.  Last date of examination if necessary .

11.  Leave sanctioned from the competent authority.




G)    Retention of the Additional Post:-

1.      Forwarding letter from HM / TIC.

2.      MC resolution mentioning continuous and satisfactory service.

3.      03 years enrolment class & section wise.

4.      Copy of post sanctioning memo.

5.      Approval of appointment.

6.      No litigation certificate / validity of MC.

Excepting above cases, if there is any other case, papers are to be required according to status.

** N.B: - Required document list may very from one D.I office to another D.I office.

The main features of the Scheme are :
(i) The scheme will have basic health cover for secondary and tertiary care upto Rs. 1.5 Lakhs per annum.
(ii) There will be no cap on the family size.
(iii) Critical illness like, Cancer, Neuro surgeries, Cardiothoracic surgeries, liver diseases, blood disorders etc. will be covered upto Rs. 5 lakh.
(iv) All pre-existing diseases will be covered.
(v) The entire premium will be borne by the State Government with no contribution from the beneficiaries
5. The scheme will be implemented by the Department of Health & Family Welfare, Government of West Bengal. The details of the scheme will be finalised by the Health & Family Welfare Department in consultation with the Finance Department.
